This indeed did come with the catalog (which I shouldn't have thrown away). It was late 97 or early 98. I could be wrong, but they may have charged a buck for the catalog. To the best of my knowledge, this was the only one that came with their mailorder catalog. I'd be curious if anyone knows anything else.
