Has insert with lyrics on one side, photo on the other. Vinyl and insert are in nm condition (in my unprofessional opinion), vinyl itself is glossy and looks great. Sleeve is very good, only a small crease in the bottom right corner under the band's name. Picked it up on a whim, thought an Offspring collector might be more into it than me.
