Sure there's all the stuff listed above, but the overall movie is excellent. If you pay attention and not watch for the sake of shock value, there is a story behind all that madness. Also if you've read De Sade's book version, Pasolini's film adaptation is nothing short of a G rating compared to the text that is the novel. Watch it, but try to remember that there is a reason for the violence and sadism. All this does serve a purpose by giving insight to the whole post war Italian fascism era. Its intentions were not meant to shock as they were to show you the harsh depravity during that era. I may need to watch this again. it's been a few years. All of this is true, however i think it's easier said than done. It's hard for the focus to be anything but the violence and sadism because it's depicted as realistically as it is. Irreversible has a story and it's tragic and upsetting in a real way. There are some scenes which will make you want to take a shower, but they are instrumental to the plot. Holy shit dude.I was just about to add this movie to this thread because of it's discomfort. My friend showed a couple of us this movie. It's told in reverse, so you see all this nasty revenge stuff go on, but you have no idea why until the most uncomfortable scene in movie history happens. It also apparently has subconcious noises come through at times of the movie and a disconcerting soundtrack to get you physically uncomfortable as well. When they showed it at Caen or whatever film festival before it's release, people were walking out sick to their stomachs.
