A rough guesstimate of shipping to the USA is £38.30 with no insurance and no tracking or £43.05 with no tracking but insured (and this is before Paypal fees). The box weighs more than 2kg, and 2kg is where UK postage starts to go up a lot. Edit - if there's enough interest, I will have time to sweep up some/all remaining copies tomorrow (Sunday).
